New Patient Information
If you are a new patient, WELCOME! We are excited to get to know you and your family, and hope that you experience a wonderful spirit of love and care as we do our very best to serve you and help you reach your goals.
Our facility is a bright, welcoming space with large treatment rooms and is located in the Emerald Plaza office park on North Walton Blvd in Bentonville.
We are a small practice with a big heart.
COVID-19 POLICY: Flight Speech Therapy, LLC takes COVID-19 and other communicable diseases very seriously. We take every effort to make you and your family feel safe and comfortable during your therapy sessions. We use a CDC-approved cleaning solution to wipe down high-touch surfaces between each session. We utilize full-strength hand sanitizer before and after each session, along with a strict hand-washing protocol. Our employees don appropriate Personal Protective Equipment throughout each session. We thoroughly clean and sanitize any shared oral motor tools and equipment (which is rare, as each patient is provided with his/her individualized tool box in which to keep and use their personal therapy tools).

Please take the time to complete the forms for the appropriate age group. Those with completed forms will be eligible to reschedule for earlier appointments as cancellations arise.
